Email notifications for form responses are not sent to form subscribers on form submission, even in case the notifications are properly configured. An error is logged in the logs.

This is issue is caused if the forms notification template id in the FormsConfig.config file located under ~/App_Data/Sitefinity/Configuration folder does not match the template id in the sf_notif_msg_template table in the database.

In order to resolve the issue perform the below steps:

1. Open the sf_notif_msg_template table in the database
2. Execute the below SQL script against the database in order to find the entry for the forms notification template

   SELECT * FROM sf_notif_msg_template
    WHERE module_name='Forms' 
      AND subject LIKE '%New form entry is submitted%'

• Copy the id of the returned entry  
• Open the FormsConfig.config file located under ~/App_Data/Sitefinity/Configuration folder
• Find the <notifications> tag and make sure that the formEntrySubmittedNotificationTemplateId property is set to the id you have copied from the database in step 3:

• Save the FormsConfig.config file
• Build the solution
• Restart the application in order for the changes to take effect
